Position Summary:

We are currently in search of a Screener/Navigator who will be responsible for conducting Health- Related Social Needs (HRSN) screenings, eligibility assessments, and navigation activities for Medicaid Fee-for-Service and non-eligible Managed Medicaid members.

Minimum Qualifications:

• High School Diploma required.  
• Minimum of 1-2 years of experience with experience in healthcare, case management or patient advocacy.
• Bilingual Spanish/English is required.
• Must have excellent communication skills and ability to provide exceptional experience to patients.
• Understanding of phone etiquette and standard office procedures.  
• Ability to thrive in a fast-paced environment.  

Key Duties:

• Verify patients eligibility for health related social needs (HRSN) screenings via UniteUs and EPACES.
• Provide a minimum of 3 attempts to outreach each patient. 
• Document outreach in alignment with required frequency, modality and timeframe. 
• Manages patient consent and attestation as required throughout the engagement. 
• Conducts HRSN screening using the Accountable Health Communities (AHC) screening tool to assess member HRSNs. 
• Ensures all questions are completed on the AHC HRSN screening tool. 
• Enters completed screenings into the appropriate SCN IT platform. 
• Develops a care plan to document an eligible members needs and why the HRSN service is needed. 

Starting Compensation:  Minimum $40,000 per year; Maximum $40,000 per year

Work Environment Type, Location, and Schedule:

Applicants must be comfortable working in an intermittent work environment primarily from home and onsite when needed, based out of the Bronx and have availability to commit to a schedule of 40 hours per week, working Monday - Friday between the hours of 8:00am - 6:00pm.
